A referral pathway designed for useful clinical work

Send us the reason for referral, not just a service label. We will clarify the clinical question, assess the relevant function and communicate recommendations in a form the care team can act on.

A helpful referral includes

  • Client name, date of birth, address, phone and preferred language
  • Primary diagnoses, recent hospital admissions and relevant precautions
  • The functional problem, change or risk prompting referral
  • Current mobility, falls history and key equipment
  • Decision-maker, consent and relevant family or carer contacts
  • Funding pathway, service authorisation and invoicing requirements

What you can expect

  • Triage to the most appropriate discipline where possible
  • A focused assessment linked to the referral question
  • Practical recommendations, including when another discipline is needed
  • Reports for rehabilitation, risk, equipment trials and funding decisions
  • Communication about material changes, barriers and review points

Funding and service arrangements

We work with private clients and a range of third-party funding and care arrangements, including Support at Home providers, plan-managed or self-managed NDIS participants, TAC and community organisations. Eligibility, authorisation, rates and reporting requirements vary; please send the relevant service approval with the referral.
